Introduction to Gynecomastia Surgery

Male breast development, or fullness in the breasts (also known as gynecomastia), is a condition that affects many teenage boys and men. Male breast development can be due to hormonal imbalance, weight gain, or genetics; even male athletes can suffer from this condition if they have used anabolic steroids. Several factors may lead to the development of gynecomastia, and the condition is quite common. Male chest contouring techniques can reduce excess fat and/or glandular tissue that is causing an enlarged breast appearance, ultimately giving the chest a flatter appearance and helping men feel more confident about the way they look. Dr. Michael Keyes provides cutting-edge solutions for men seeking to address this common concern with Gynecomastia Surgery in Miami that can effectively address this problem with successful results.

What is Gynecomastia?

Gynecomastia is a common medical condition characterized by the enlargement of male breast tissue. It can occur in one or both breasts and is caused by an imbalance in hormones, particularly an increase in estrogen levels relative to testosterone. Gynecomastia can affect males of all ages, from infants to older adults. While it is not a severe health concern, it can still lead to emotional distress and self-consciousness in affected individuals.

Treatment options for gynecomastia include surgical procedures such as liposuction or Male Chest Contouring surgery. If you feel your life is affected by this condition, then it’s time to reach out to Celebrity Plastics and take the first step toward the male chest contour that you deserve.

Stages of Gynecomastia

There are four grades of gynecomastia, which are defined as:

  • Grade I – Minimal enlargement without excess skin
  • Grade IIa – Moderate enlargement without excess skin
  • Grade IIb – Moderate enlargement with excess skin
  • Grade III – Marked enlargement with excess skin

Not each stage of gynecomastia will demand surgical intervention – and those that do are not always served by the same procedure. For example, men experiencing Grade I gynecomastia may find success through enhanced exercise and a sensible diet, both of which may improve conditions in men with Grade IIa gynecomastia as well. However, Grade IIa can also be addressed with liposuction in cases where fat fails to respond to diet and exercise.

Liposuction may not encourage enough skin tightening for Grades IIb and III. In these cases, tissue and skin excision may be necessary for a complete restoration of form. This is entirely dependent on factors unique to your body and desires and can be discussed in greater detail during your consultation with Dr. Keyes.

Types of Gynecomastia surgeries

  • Liposuction
Liposuction is most effective for removing fatty breast tissue, but more dense/tough breast tissue may be removed with the addition of Liposuction techniques. This technique uses PAL, or Power Assisted Liposuction, in cases of mild to moderate gynecomastia with minimal excess skin. Using liposuction, there is less compromise of blood supply, nipple distortion, and areola slough. In addition, post-operative complications are fewer with this technique than with open surgical resection; however, liposuction is usually not effective for correcting moderate to severe glandular gynecomastia. The fatty and glandular components of your breast must be assessed before surgery.
  • Liposuction with direct glandular excision
The dense/tough tissue below the nipple usually requires direct surgical excision and often cannot be removed with liposuction alone. In some cases, fibrous glandular tissue causes an enlarged breast. Dr. Keyes can make incisions to remove this tissue. In the “pull-through technique,” a very small incision (approximately 5 mm) is made at the areolar edge. Following Liposuction, the glandular tissue from the overlying areola is pulled through the incision. The major advantage of this procedure is a smaller incision.

Direct excision may also be required in some cases, if there is excess skin to be removed or if the size of the areola must be adjusted. Most excision is performed through a small incision at the lower border of the areola only, but incision patterns must be adjusted based on the needs and goals of each patient.

  • Male breast reduction / Male mastectomy with free nipple transfer for severe cases
Many individuals who have lost a lot of weight experience excessive amounts of loose skin in the breast area and have pseudo-gynecomastia. A “full” male breast reduction may be the right option to address this concern. Dr. Keyes often utilizes a pedicled gynecomastia surgery technique, also known as a “pedicled chest lift,” to perform this treatment, which can preserve the blood supply to the nipple, along with an IMF incision (crease incision) and a full periareolar incision (around the nipple). This is typically the best procedure for patients with significant weight loss and loose skin; however, some patients may opt for less invasive treatment. The free nipple grafting technique is a commonly performed option for patients who have lost large amounts of weight and have pseudo-gynecomastia. Free nipple grafting allows Dr. Keyes to place incisions around the areolas and in areas that should yield less conspicuous scarring when compared with techniques such as anchor lift gynecomastia surgery. The free nipple grafting method can be ideal for patients with particularly severe gynecomastia after major weight loss, helping to address the condition successfully while dramatically reducing the potential for prominent scars.

Teenagers up to middle-aged men enjoy the improvements they can obtain from gynecomastia surgery, as well as their improved appearance both in and out of clothing. If you are suffering from gynecomastia, please don’t hesitate to reach out to our practice to learn more about your options.

What’s the First Step?

If you are considering Gynecomastia Surgery in Miami to address enlarged breast tissue and achieve a more defined chest, the first step is to meet with our Patient Care Coordinator who will discuss your aesthetic goals and help you determine if this procedure is right for you. If you wish to move forward, they will schedule you a consultation with Dr. Michael Keyes.

During this initial meeting, you will have the opportunity to discuss your concerns, goals, and medical history with Dr. Keyes. It is very important to be open and honest during your Gynecomastia consultation so that you can have a safe and effective surgery. Dr. Keyes will discuss the results of the workup into the cause of gynecomastia and will recommend the type of gynecomastia surgery to perform, based on the severity of your gynecomastia

Once Dr. Keyes has evaluated your case, he will recommend the appropriate course of action tailored to your surgical needs. If you decide to proceed with Gynecomastia Surgery, our Patient Coordinator will help you schedule your procedure date and provide you with some pre-op instructions that will ensure the best possible results.

What to Expect on the Day of Gynecomastia Surgery

On the day of your surgery, you will meet with Dr. Michael Keyes once more to review your procedure one more time. He will make the necessary marks that will help guide him throughout your surgery, as well as give you a better idea of what your results will look like. You will also meet with our Anesthesiologist who will administer your anesthesia so your Gynecomastia surgery is more tolerable.

Once you are sedated, Dr. Keyes will carefully make incisions in inconspicuous locations to access the excess breast tissue. Depending on your specific needs, liposuction may be utilized to remove excess fat from the chest area. Dr. Keyes will then sculpt the remaining tissue to create a more masculine contour, ensuring a natural and symmetrical appearance.

The surgery generally takes 1-3 hours depending on the severity of the disease. It’s important to have a trusted companion with you to drive you home safely post-operation.

Contact

The first step to getting your Male Chest Contouring surgery is to contact Celebrity Plastics and schedule a consultation. Before you consult with Dr. Keyes, you will need to meet our Patient Care Coordinator to discuss your aesthetic goals and eligibility.

Consultation

When you come to Celebrity Plastics for your consultation, Dr. Keyes will evaluate your health and explain which surgical techniques are most appropriate for you, based on the condition of your chest. Once cleared by Dr. Keyes you will talk to our Patient Care Coordinator who will pick a surgery date with you after an initial deposit.

Pre-operative preparation

Before your Gynecomastia surgery, you will be required to get some labwork performed, such as a blood test and an EKG, to ensure that there are no underlying health issues that create unnecessary risks. We do offer these tests at our facility for a small fee. You should also supplement your diet with high-protein shakes in the days leading up to your procedure. You are also strongly encouraged to maintain a healthy lifestyle, which includes quitting smoking and consuming alcohol. You will also need to have your prescriptions filled before your surgical day and bring them in with you.

Surgery

When you arrive, Dr. Keyes and our Anesthesiologist will speak with you to go over the details and answer any lingering questions you may have. Dr. Keyes will also take this time to make any markings he will use as a guide for your procedure before sending you to the Operating Room. The operation should take about 1-3 hours depending on the techniques used.

Recovery

After you are released from our care, you will be able to return home and recover comfortably. You can expect some swelling, bruising, and mild pain during your initial recovery. You will need to continue taking your antibiotics until finished, even if you are feeling better, as well as high-protein supplements. This will ensure you recover as successfully as possible. Moving around as much as possible without causing distress is very important as well, with most patients returning to work and other daily duties within seven to 10 days. You should not exercise or perform any strenuous physical activities for up to four weeks.

Post-operative appointments

The day after your Male Chest Contouring surgery, you will need to return so a nurse can observe your initial healing and change any bandages if needed. Dr. Keyes will then have you return about a week post-surgery so he can monitor your recovery and satisfaction. You can expect another appointment about a month or two later, followed by another one three months post-surgery, then again six months later.

Gynecomastia Surgery Recovery

After Gynecomastia Surgery, you will be able to leave to recover at home in comfort. We advise all cosmetic surgery patients to have a ride ready from a family member or friend as they will still be under the effects of the anesthesia.

You can expect a recovery period of about one to two weeks. Initially, you may experience some swelling, bruising, and discomfort, which can be managed with prescribed pain medication and wearing a compression garment. It’s essential to follow post-operative instructions, including avoiding strenuous activities for a few weeks and attending follow-up appointments.

If you undergo a more invasive procedure, such as a free nipple graft, you can expect a longer and more involved recovery. Immediately after this surgery, your body will develop new blood vessels that grow into the graft. To ensure the graft has solid and continuous contact with the body during this process, a ‘bolster’ dressing is placed on the graft and tied down to the skin with sutures. When you come to see Dr. Keyes about 1 week after surgery, he will remove the bolster if he believes your grafts are healing well. During the first one to three weeks after surgery, the graft will continue to heal, becoming quite resilient after 21 days. It should take about six post-surgery for the grafts to fully heal and have similar strength to the non-grafted tissue. It can take up to a year or more until the final healing of nipple grafts is complete and the redness around the areolas has disappeared.
